Cerebrolysin

Independent reviews find little clear benefit after stroke.

Not a single peptide but a purified extract of pig brain, given by infusion. It is widely used for stroke and dementia in Russia, Eastern Europe and China. Independent reviews are much less positive than its reputation there. Made by EVER Neuro Pharma, also called Pig-brain peptide extract.

A mixture of many short chains, drawn as a sketch.

What the evidence shows

Acute stroke
Failed
A 2023 Cochrane review of seven trials (1,773 people) found no effect on deaths. No trial reported how many people ended up dead or dependent on others.
Vascular dementia
Early
A Cochrane review of six trials (597 people) found a small gain in thinking scores that may be too small to matter, with very low certainty.
Alzheimer's disease
Early
Pooled results from small, maker-sponsored trials suggest a benefit.

Side effects and risks

Serious
In stroke trials, non-fatal serious adverse events were about twice as common as on placebo. Serious events overall did not differ.
Common
Headache, dizziness and agitation.
Unknown
Made from animal tissue.

Who should avoid it: Self-injection. It is a hospital infusion.

How it works

Where it acts
A mixture of brain-derived peptides said to act like nerve growth factors.
What it changes
In lab and animal studies, protection and repair of nerve cells.
What people notice
Given by drip over weeks. Independent reviews find little clear benefit after stroke. Dizziness and headache occur.

How it is given

Into a vein in a clinic. Stroke trials used 30 mL a day for 10 days. Products sold to swallow are broken down in the gut.

Supportive expert view

Argued that Cerebrolysin should be treated as an evidence-based therapy for routine use in acute and subacute stroke care, pointing to its inclusion in Austrian, German, Canadian and European neurology society guidance.

Cautionary expert view

Calls the craze unfounded. He found no randomized human trial showing the advertised benefits for any of the unapproved peptides he reviewed, and warns about stacking several at once. He points to animal work in which TB-500 sped up dormant tumor growth and to a theoretical cancer concern with growth-hormone peptides and BPC-157. He treats the GLP-1 drugs as the well-studied exception.

Led the 2023 Cochrane stroke review. Said the drug is widely used in Russia, former Soviet states, Eastern Europe and China, found no benefit, and said her team would not recommend routine use in acute ischemic stroke.

Harm reportedtrial result in rare group - 1773 people - 2023

The 2023 Cochrane review of 7 stroke trials found probably no effect on death. Non-fatal serious adverse events were probably more than twice as common with the drug. Reviewers rated two manufacturer-supported trials at high risk of bias.

Cerebrolysin

Evidence review: Cerebrolysin is approved in many countries but not the US, looks safe short term, and shows small cognitive gains but weak evidence.

Rare independent evidence summary, though dated 2016

When options run out

Uses people turn to when standard treatment has failed.

Add-on to stroke rehabilitation

Who
Patients with moderate to severe ischemic stroke in countries where the drug is licensed.
Evidence
The manufacturer reports a 2021 European neurorehabilitation guideline backs early add-on use. The 2023 Cochrane review found no benefit in acute stroke and more non-fatal serious adverse events.
Route
Licensed prescription use in parts of Europe and Asia

Source: Manufacturer news page, 2021-07

Moderate to severe traumatic brain injury

Who
Hospital patients with brain injury, where few drugs aid recovery.
Evidence
Two small placebo-controlled trials pooled to 185 patients showed a small to medium gain on combined functional and cognitive measures at 30 and 90 days, with safety matching placebo. Larger independent trials are lacking.
Route
Licensed prescription use in some countries

Vascular dementia and Alzheimer's disease

Who
People with dementia who have exhausted standard drugs, mostly in Europe and Asia.
Evidence
A Cochrane review of 6 trials in 597 people with vascular dementia found better cognition and general function scores, but at very low certainty and possibly too small to matter. A charity review found modest Alzheimer's signals not confirmed in large trials.

Long COVID brain fog

Who
People with lasting post-COVID cognitive symptoms looking beyond rehabilitation.
Evidence
A 2024 systematic review of 17 studies on brain fog treatments in long COVID included no Cerebrolysin study. No controlled evidence was found for this use.

Legal last-resort routes

Expanded access (compassionate use)

For a serious or life-threatening condition with no comparable alternative and no trial you can join. Your physician asks the company for the drug, then files with FDA and an ethics board. The company can refuse. Costs are often not insured.

Source: FDA, expanded access for patients

Right to Try

For a life-threatening condition after approved options are exhausted and no trial is open to you. The drug must have finished Phase 1 and be in active development. Your doctor asks the manufacturer directly. FDA does not review requests.

Source: FDA, Right to Try

Trial finders

Search ClinicalTrials.gov by condition or drug name, check eligibility and call the listed site. ResearchMatch is a free NIH-funded registry that lets researchers find you. Discuss risks with your own clinician before enrolling.

Personal import for serious conditions

FDA may not object when a US citizen imports up to about three months of a drug for a serious condition with no effective US treatment, with written personal-use confirmation and a US doctor's details. It is discretion, not a right.

Source: FDA, personal importation

Patient-specific compounding

When the approved product cannot meet a specific need of an identified patient, a prescriber can order a compounded version from a licensed pharmacy. Lower price alone does not qualify. Ingredients must be pharmaceutical grade.

Is Cerebrolysin FDA approved?

No, it was never approved by FDA. There is no EU-wide approval, but it is authorized nationally in some member states, including Austria.

What are the side effects of Cerebrolysin?

Headache, dizziness and agitation. In stroke trials, non-fatal serious adverse events were about twice as common as on placebo, though serious events overall did not differ.

Can Cerebrolysin be taken by mouth?
